Important note: Transcripts are not considered official unless they have been sent directly to the Undergraduate Admissions Application Centre by the institution concerned. An unofficial copy of your student record can be submitted in the interim. If your records are in a language other than English or French, please arrange for the submission of a certified translation in either English or French in addition to the official transcript in the original language.

Academic Assessment Forms/Reference Letters

Please note that academic or business reference letters submitted with the academic assessment forms must be written on the institution's official letterhead and bear an original signature. Wherever possible, the academic assessment forms and letters of reference should be submitted directly to Concordia University from the institution, in the institution's envelope. In the event that the letters cannot be mailed directly to us, the envelope must be sealed and the referee must sign his/her name across the flap of the envelope.
We are missing all three of your reference letters.
English Proficiency Results

Scores of the Test of English as a Foreign Language (TOEFL) or University of Michigan English Language Assessment Battery (MELAB) or International English Language Testing System (IELTS) are to be sent directly by the examination board. Concordia University's institution code is 0956.

It is recommended that you provide the requested documents by the deadline above in order to ensure the timely assessment of your file.
You may return the requested information either in person to the Birks Student Service Center, 1400 de Maisonneuve Blvd W., Room 185, or by mail to the address listed at the end of this form. The MyConcordia Student Portal can be used to view the status of your admission application and to upload additional documents you may need to add to your file. This can be done by going to the link "Application Center" which you will find on the menu in your student portal.
The majority of programs in the Faculty of Fine Arts as well as some in the Faculty of Arts and Science have additional requirements such as a portfolio, letter of intent or an audition. Detailed information is included in the application material and on the faculty web pages. It is the applicant's responsibility to have fulfilled any such departmental requirements by the stipulated deadlines.
