To request a change in the name or date of birth that appears in your TOEFL® online account, fax a copy of your primary identification (ID) document, with your photograph clearly visible, and a copy of this email to +1-610-290-8972 within the next 3 business days. Be sure to include your ETS ID number, registration number and case ID number on the fax.

Please allow 10 business days after you send your fax for us to review your documentation and update your account. After the 10 days, log in to your account to verify that your information is correct.
Keep in mind that the name that appears in your online account must exactly match the identification document you will bring to the test center. If the names do not match, you may not be able to take the test, your registration will not be rescheduled and your test fee will not be refunded.
