Every recommendation letter should have a general format:
1) First and introduction in which the recommender says how and in what capacity he knows the applicant (For example, he passed X with me in the Fall of 2008)
2) The recommender then explains about the applicant's performance in that course(s) and maybe even his grade relative to others. Words like hardworking, industrious, diligent, etc. are often used.
3) The recommender could comment on personal traits of the applicant, such as politeness, good social interaction, positive outlook, etc.
4) The LOR should be concluded with a paragraph in which the recommender recommends the applicant to that graduate program. For example, "Considering the above, I would highly (or strongly) recommender X for graduate studies at your department."
5) At the end of the LOR, the specifications of the recommender should be given, such as name, position, dept., institution, web page address, tel number, email address.
